I can only go based on my experience with Virgin Mobile, but they require no information at all to set up a phone, and you get a number from the area code where you activate your phone (unless you're transferring a phone#). It may be different with Sprint, but it might not require more than a credit card or bank account for billing.

I would seriously consider Virgin (best rates by far) until I saw the coverage map. The city is covered and close around some major roads. Its basically the core Sprint map without the extended range that covers a better range. The problem I face is that I may or may not have to do some rural work at some point, or driving a few of these roads.

I've been content with a prepaid phone from Verizon, but I have extraordinarily low mobile phone requirements. I don't think I had to have a fixed address for it, but at this remove from the purchase date I don't remember. I did get a rebate for my phone that covered just about a full year's worth of service, but paid for the actual phone in full too.
