summary: he doesn't know what it means, so it clearly can't be an insult.
Lovino does not know what the word "faggot" means- he doesn't even know what "gay" is, why that word is an insult- but the other boys call his brother one. Maybe it's because he's always with the tall, blond boy that never smiles, or because he's always so terribly happy.
But Lovino knows, by the way that the tall boy frowns and yells at the other boys, that it is not a good word ad something mean that they say to his brother. Sometimes, his brother even cries.
And, as much as he hates the tall boy, he likes him because he's nice to his brother when Lovino himself cannot be.
I's uncool to hang out with your little brother, and if it means he'll get called a "faggot", too, he'll avoid being around one specific boy all the time.
Girls were gross, though. And he doesn't want to be treated like how they treat his poor, simple little brother. All Feliciano wants to do is paint and play with the tall boy.
All the other boys want to do is play-fight and play tag, and explore in the woods. And Lovino wants so badly to do it with them.
So, the next time they call Feliciano a "faggot", so does he.
And the tall boy cries.
