I mean, from the get-go I suspected it was Booth's brain tumor imagination. And it was kind of an interesting one-shot story anyway, and a very amusing use of all the characters we know. Some real knee-slappers, too. ("I'm a bar tender. I'm practically a psychologist.") Loved that Zach was back as the personal assistant.
But what a weird way to end the season. Guess they couldn't think of anything more gripping than Booth's, "Who are you?" and had to drag that out to an hour? Oh yeah, and they had to get them in bed and pregnant. *eyeroll* Like it matters to people who ship Bones/Booth if they're aren't them when they're in bed and pregnant!
My theory on him asking who she is: I'm thinking he's asking, "Are you my nightclub wife, or are you my FBI partner?" You know, he's having trouble figuring out which is reality. My guess is that he isn't saying, "I've never seen you before in my life, who are you?"
That's an interesting theory. I like your suggestion better than the alternative, for sure.
To be honest, I completely forgot that at the end of the last episode they'd discovered that Booth had a brain tumor and underwent an operation. It wasn't until the end of this episode when he woke up in the hospital that I went, "Oh yeah!"
I just thought this week's episode was an aberration. Something "fun" they decided to do, and I couldn't decide if that was fun or annoying. Especially when it was the season finale.
I know a large part of the ambivalence I'm feeling is due to the spoilers and commentary I'd read, like:
Bones fans were delighted earlier this year when it was revealed that -- after four years of the will-they-won't-they dance -- Booth and Brennan will hit the sheets in the season finale. The powers that be have been very clear that the Brennan/Booth sex isn't a dream sequence or a hallucination.

Hmmm. Unless... (highlight to read) Booth is obviously attracted, and emotionally connected, to her. Just coming out of anesthesia coma, it's really believable that (because of his real feelings) he'd be genuinely confused about which was reality, because he'd probably want the marriage more. Now that I ponder it, it does seem evident something occurred within her as well, when he didn't wake up. All season long (I'm sure you've noticed) there have been little bits of "You love Booth" to which she replies, "I DON'T LOVE BOOTH," and Angela says, "We all love Booth, we care about him," and Brennan is totally sublimating any emotional attachment below her rational connections to him. Perhaps she's now realized the emotional component of their connection. It certainly looked like that had begun before his surgery, when she was tearful, though she was still fighting to be rational as well; but there she was with

Here's what Booth says at the end, when he comes out of his 4-day coma, in spurts between what Bones says to him: (1) It was such a weird dream. (2) So real. (3) It felt so real. (4) Who are you?
He's not really responding to her, so I'll just discount what she says altogether.
But upon second viewing, I think it's pretty clear that each of them had an experience. Each of them had a story. She wrote one, he dreamed one.
There are two stories in the episode. The first is that first minute-ish of the ep, narrated by Hodgins, where she comes "home" (whose home?) at 4:47 and gets into bed with Booth and they make love. The second story begins with them asleep at 05:43 and the Cam/Jared knock and is the rest of the AU episode
