I can't see how anyone could possibly put on a drumhead and not notice that sitting in there. He had to either be in a big hurry so he wasn't paying attention to anything except the drum screws, or it was dark wherever he did it. And if he moved the drum at all while putting on the head, the wrench would have slid around. I think he either did that on purpose, or he just really wasn't paying attention. That's my 2 cents.
