Is it possible, though? I think it’s a bit like asking Sherlock to put aside the detective. As much as John would love to be able to do it (and I understand why Sherlock is asking for this), being a doctor is a big part of who he is.
“John,” she said softly, curving a hand on his arm. “I’d like to help you. Be…a friend.”
I believe I mentioned, back when you were posting BHN, that if Irene stopped trying to manipulate and hurt him, they could be great friends because she was so similar to Sherlock. In this situation, she could act as a buffer when things get tense between John and Sherlock and be the one to look after Sherlock when John is unavailable or when he needs some time alone.
John stared into the camera, nodded. “Yes. Now what the bloody hell do you want?”
A beat, then one word.

I don't think John can stop being a doctor, no, but I do think there's a line that can get crossed with lovers who try to caretake too much and it can mess up the relationship. Their relationship has been based on John taking care of him to some degree, but doing physical exams when you're also the person who is a physical lover...that is hard on the sick person, I think. I think that's what he's addressing more than his concern or tendency to check up on him in a more general way.
Irene has changed a lot here, and she will change more. What happened to her was a helluva way to learn it, but I think she did!

It was quite a shocking chapter, not just physically for Sherlock and Iarla, but for all the anger and frustration and sheer helplessness being felt by everyone. It’s especially distressing to see Mycroft frustrated - the omnipotent Mycroft who can be relied on to “solve” everything. But it’s also interesting seeing the interaction between the characters growing, mellowing even in the case of John and Irene.
”In John, kindness would always win out over pique.”
Of course, and it’s amazing that Sherlock can recognise that even in his own extremis.

Now that really is a scary cliffhanger.
