i went through the same bumpy ride you did, and all i can say is, you have to talk to him about it, if you haven't. there's no way to make this better unless he knows how you feel. and then it's up to him to do his part.
at the same time, of all the things he has done or not done to upset you, you must pick out those which are important. i learnt from conversations with my guy friends who are in LDR too, and i see things from their point of view. so it helps.
