this icon from my current batch. This does involve adjustment layers (2 curves, 1 hue/saturation, 1 color balance), but is mostly color fill layers.

I got the basis for these steps from

Step 1
Crop and resize your picture. I used a cap from
marishna prison break screencaps from episode 1x20.
Then prepare your base.
Go to Image/adjustments/Variations/ Make sure you click "original pick" to make sure it is the current coloring, and then click lighter once.
Then go to image/adjustments/hue and saturation and increase the saturation by +30
Step 2
duplicate base and set to screen at 100%
Step 3
Color fill layer #c2e4ee set to Color Burn at 100%
Step 4
Color fill layer #e0f0f5 set to Multiply at 100%
Step 5
Duplicate your base and bring it to the top and set it to Screen 100%
Step 6
Here is the tricky curves layer that I use for pretty much all my icons.
New adjustment Curves layer set to Soft Light at 100%
RGB: I: 106 O: 150
Red: I: 115 O: 137
Green: I: 82 O: 143
Blue: Nothing
For those that can't get this working, you can also download the curves layer
here. Please try to get the points yourself, because it is part of a learning process :)
ETA: Now on this layer, it doesn't work right for everyone for some weird reason, so in order to make it work you can also go to image/adjustments/curves and apply the curves layer directly TO the image instead of having it a separate adjustment layer.
Step 7
Color fill layer #011637 set to Exclusion 70%
Step 8
Duplicate your base and drag it to the top. Set it to Soft Light at 100%
Step 9
New adjustment layer: Hue/Saturation. Saturation: +30 Lighting: +5
Step 10
New adjustment layer: Color Balance. Color Levels: +9, 0, -10

Color fill layer #e1e1e1 set to Color Burn 100%
Step 13
Color fill layer #c8b3ae set to Overlay at 32%
Step 14
This layer was a bit tricky but fun. I created a new layer and set it to soft light. I then took a royal blue #0000fe with a round brush and painted the background and over her sleeves.

Then add some tiny text, this brush was from
_chokeanddie.
