Based on what the article said, it sounds like she used an auto-locking carabiner. That's a big "no-no" in aerial and also in rock climbing (for the anchor) - at least from my perspective. I don't like those auto-locking biners as I don't trust them to always lock properly. If she used a manual locking carabiner, then you can't lock it if it's not shut properly. So she either used one of the auto-locking ones or she used a manually locking one and didn't lock it (maybe because she was nervous or rushing??). I can't imagine she'd use a biner without a lock on it at all. Seems crazy to me.
