Looks to be an advanced case of epidermodysplasia verruciformis, which is a rare disease characterized by extreme susceptibility to cutaneous herpes papillomavirus infection and squamous cell carcinoma. Lesions develop in childhood with malignant transformation (especially of sun-exposed areas) during adulthood in one-half of the patients.
