I would have the vet check and clean his teeth professionally, though it's expensive. He could have a rotten tooth that has got abscessed if his breath is that bad, and that can be dangerous in humans who have significantly more space than a cat does between their jaws and their brains.

Rescue-dog-girl Kaiya had surprisingly bad breath until we got her teeth checked during cleaning - she had not one but two broken and abscessed teeth which have been removed. This had to be discovered and dealt with by the vet; I was brushing her teeth, but that's "health maintenance / problem prevention" and won't take care of already-developed problems.

I try to brush the dogs' teeth at least twice a week if not every other night (and Kaiya's breath is fine now.) As far as I know, dogs and cats only really need the outer sides of their back teeth brushed regularly, so it doesn't take too long. Our vet tech recommended using a very small kiddie toothbrush instead of those "for pets" brushes, which is easier to slip into the cheek pocket (primed with tasty pet toothpaste, of course!)
