Oooh, I like your theories! I wondered about Jared's comment that Sam has been in hell "a couple of times" and just chalked it up to him being chatty or else confusing hell and heaven. But maybe there was more to it.

Also, if I read the same spoiler, it said that Samuel was not pulled from Heaven. So was he in hell? What's up with that? Hmmm....food for thought!
