Can a friendship that includes intimacy be  NOT a relationship?  Does that even make sense?

You mean two people who are friends who enjoy having sex with each other but do not have a romantic relationship?

It's possible. I admit it's not an ideal setup, and one I've never done, but it's possible.

No. We're all in relationships with each other. You and I have a friendship relationship, see definition below. However to have a friend, with or without sex is a relationship and comes with responsibility. Sex however makes it no more and no less and this is my not so humble opinion.

re·la·tion·ship
1. a connection, association, or involvement.
2. connection between persons by blood or marriage.
3. an emotional or other connection between people: the relationship between teachers and students.
