I don't know that a failed PT test can be removed even with an injury that is found after the fact. Check with someone in personnel. It sounds like your 1SG has already made up his mind.
At least in the Navy, you can take a PT test with events waived (no run if you have a leg injury, no push-ups if you have a bum shoulder, etc...) if cleared by medical.
Soldiers with permanent profiles can take an APFT, subject to the limits of their profile; it says so right on the form...just like it gives the reference regulation.
...but I'm feeling generous, so here's some help.
AR 40-501, para 7-11 "Preparation of the DA Form 3349"
(f) Item 6. Physical Fitness Test. Check either “Yes” or “No” to indicate whether the Soldier can perform the activities for the APFT. The “Yes” or “No” blocks on the alternate APFT need only be completed if the Soldier has restrictions for the regular APFT. If the Soldier cannot perform at least an alternate APFT the profile serial will be at least a 3 and referred to an MEB.
