Every dude I know who wears a utilikilt goes commando underneath. I know of no exceptions. It's considered one of the perks of wearing one. A couple weeks ago, I even saw a guy tenting his utilikilt.

As for the straight guy...I dunno. In this day and age, there are more and more straight guys who are comfortable enough with themselves, so as to playfully flirt with gay guys. I think they like the attention, in the same way that women like attention from men, but still enjoy being unavailable.
