Have you tried calling your ISP? They will be able to test the connection and tell you straight away whether it is a problem on their end or a hardware issue. They would have some idea of common issues and suggest what might be wrong at your end, although of course their responsibility does not extend to sorting that bit out for you, but at least you would have a better idea.
From the sounds of it, I'm also leaning towards something being wrong with your modem. Can you hook up the modem to another computer, a friend's for example, and see whether it works there? Sorry to hear about that. Internet problems suck.
