I've never quite thought of it that way, but it seems so very true...and so very humbling! How much different would the world that we live in today if each of us judged others in the same manner in which we judged ourselves?
I think that a missing ingredient in your recipe is that you're always aware of your own intent, you can't judge others by anything other than their actions. Furthermore, I try to judge myself based on my actions too. There is an old saying, don't know if you're familiar with it or not but it goes like this:

Personally, I agree with the other commenter that life would be "better" if we judged others by the same standards we judge ourselves. Unfortunately, I believe that ACTIONS should be that measuring stick not INTENTIONS.

you can't judge others by anything other than their actions I can see your point, in the literal sense, but if you truly judge others by ONLY their actions, you are missing out. I try, and fail many times, to think about why the action was done. IOW, understand the intention. It is amazing how many possible reasons there are for doing one thing. You may find out that something that ticked you off was actually an incredible act of tender mercy being extended to you. This has happened to me in the past.

I don't believe "The path/road to hell is paved with good intentions" has anything to do with the actions taken being good or bad. At least that is never how I have taken it.
I've always took it to be speaking of a failure to act.
That is to say, lots of people have good intentions and PLAN to do all sorts of things, but they never do anything about it. What good are intentions if you don't ever act on them? You could reason that they are actually selfish and evil if you profess to want to do good, but continue sitting on your rear.
Ah yes. I spaced the original context. I put the quote into a different context(post action) due to the thought of judging others actions by intentions. Another prime example of failure to understand your intention in using the quote(the common use I must agree). And now I know the intended purpose of the quote. Too bad I judged first. ;)
Still, the more I think about it, the more I think if you continually have good intentions, eventually you will act on them, or stop having them, or at the very least stop having very many of them; then you are on the steep part of the road to hell.
