It seems to me that when JARVIS addresses or refers to a person, the title and name are used (Ms. Potts, Agent Coulson...) Here's what I need to know

1) I'm pretty sure I've heard JARVIS refer to Tony as "Mr. Stark" when relaying information.
2) I think JARVIS would refer to Thor as Thor. Technically, Thor is a prince but he's also a god, so there could be a lot of honorifics used.
3) Yes.
4) I think Thor would continue with his formality. He comes from a noble house where etiquette was held in high regard. I think he might say something like, "Dr. Banner" but I think that's as relaxed as his speaking would get.
