Stop asking him to lunch.

He's just not that into you.

Well, I never did see it from that POV. We go to lunch or coffee 2-3 times a week.

It's sad that I need to. Ice him/her out. Snub them--gays are good at this. And if he/she asks, then tell him/her why (as you were just complaining about the passive/aggressive thing).

I used to have a friend like that, and as hard as it was for me to do it, I just don't talk to her anymore. I'd had enough of her passive-aggressiveness (my brother had even gone so far as to call her a bitch based on his observations of our interactions over the years).

I think you were very kind to try to talk to said friend about their PA behavior, but it seems as if the lesson was lost upon your friend.

My answer to your friend would not have been 1, 2 or 3 (thought I might have thought those questions), but rather "What do you mean that the restaurant is small?"

And let him or her try to explain either that it's supposed to be private time alone with X, or whatever. Make HIM squirm instead of internalizing your frustrations. :)
