Tonight I sent in a report of someone named "LightKnightt" on Moon Guard for so obviously not being a "name". According to the ToS (and posted on WoW Wiki):

GMs don't open the tickets by server...as far as I can tell they just get whichever is next in line. So the GM might have just been like "Lightknightt? Well, nothing offensive about that. NEXT" without realizing it was breaking violation on its server.
I've also heard that sometimes GMs will talk to the person whose name is being questioned and if they feel the name has a good enough story behind it they'll leave it alone and consider it justified.
Or, I dunno...sometimes I think it takes more than one report to get names changed. I know some guy named "Brbdaterape" didn't get a name change until several of my guildies reported him (after my initial ticket). Who knows!

Maybe I'll create a little note pad file with the ToS info from WoW Wiki stating a name on a server requires being an actual NAME. :)
Often if it's something that could possibly be a nickname, I let it go. Example, if I were to see a rogue named "Slash" I could see it being a nickname. However, a rogue named "Istabu" isn't a "name" or "nickname" but rather someone being stupid.
