1. cerebral palsy
arthritis
fibromyalgia
generalized anxiety disorder
cyclical insomnia
...and that's it, I think...unless you count insanity :)

For 2, I'd make sure you were in a safe, comfortable place, get you any meds or such that you needed, and ask what you needed to get through a flare up. And then keep my eye on you to make sure it was passing :)
