i know. i use the same rules too (bottom matches nostrils, top with eyes/eyebrows). however, that's how high it is on the source picture! my best guess is that the model's ears are located higher than average, and that the photograph's angle is from above her (in that case, i didn't convey that angle well).
the shading on the neck should be significantly darker.
You're right, but let me quote myself:
"And you may notice my laziness in drawing everything below the chin." xD I did a sloppy job on everything besides the face because I considered it secondary...

I don't quite get what you mean by draw all 4 steps. I took a picture after I finished each "step". Well I normally combine steps 2, 3, and 4 together; but I decided to try splitting them up to see if it might make it easier to draw.
