OK, it's time I reported back. I saw the Doctor, and a very impressive doctor he was. So - if stress is a key component of your recent difficulties, are you considering any form of stress reduction such as yoga, mneditation, etc?

I guess my first thought has been to reduce the cause of ther stress, the workload itself. But you are right of course, meditation and exercise would both be helpful. I meditated (and exercised!) for many years but for some reason have been having trouble returning to it again.

Reducing the root cause is not always practical, sadly enough.

i had horrible panic attacks (multiple times a day - sometimes felt constant) for a couple years, and had to eventually go to the doctor because i was feeling too faint, dizzy, sick sick...nearly disfunctional. and was prescribed a psyciatrist! ended up taking a 10 weeks workshop called "mindful meditation" (w/ a bit of cognitive-behaviour therapy in there) and it saved my life. haven't had a full on attack since (and that was 2yrs ago). felt like a miracle.

Well, this attack was bad enough that I almost went to emergency, bad enough that I spent the following day in bed. I am not entirely convinced it was a panic attack but it was similar. I have meditated for many years, and am familiar with mindful meditation and other techniques... but frankly I think this is something else. Meditation is amazing though--it tuned my life around when I was in my early thirties. I continued meditating every day until I was 48. For the last 11 years I have continued meditating occasionally, but not in any structured way.

yeah, could be something else. tho i definitely had some that were seizure-like... paralyzed and shaking and mind unable to process. and it did leave me exhausted and somewhat brain-dead after. tho never had them wake me in the middle of the night... who knows. bodies are so complex. i'm glad you're taking measures to de-stress regardless. i've since stopped meditating in a structured way too, but being able to tap into that inner steadiness/peace when necessary such a sweet skill to have... etc.etc. :)
