I see someone tried to save money with a culvert where they really needed a bridge. I've seen this happen to a dirt logging road before (and watched them put in the new culvert), but never a paved one.

The culvert would have worked it they'd have put in proper measures to prevent piping along the outside of the pipe via a cutoff trench, concrete cutoff collar, or necessary compaction.
