Option 2 and option 4, in that order, assuming that option 2 can be carried out in 1 - 2 weeks.

When it comes to your health don't be afraid to get your credit card out, like I did for my back when I needed a physio after my health insurance ran out *nods* srsly don't be in pain any longer. xx

I agree with this.

I've been through a similar ordeal recently and ended up being given a three month appointment -so I had an emergency extraction instead; not what I wanted but didn't feel I could carry on for three months like that. I still have to go to that appointment as there seems to be a serious underlaying problem...

Thanks :)
But I don't want you to pay for it, I want it to be free.
Red's offered as well, bless him. But I want to do it off my own back if I'm going to do it that way. If I can not go overdrawn at the end of the month because of it. Which is theoretically possible if I work my behind off...

Healthy teeth and not being in unnecessary pain are very high on my list of priorities, just do it. You know it's not getting worse then, at least.

If it's really that bad, I'm sure 20 friends all donating you £10 rather than buying you a drink next time it's your birthday will do the job. I put my hand up to be the first volunteer. Xx
