The four-dimensional symptom questionnaire (4DSQ) is an originally Dutch self-report questionnaire that has been developed in primary care to distinguish non-specific general distress from depression, anxiety and somatization. Small amount for pharmaceuticals bs. :)

And it's not a just translation, it's linguistic validation with phychometric conponent. Big job, clinical project, performed by the team consisted of profs. Yachno, Smulevich et al., and covered by CRO.
