I'm exempt from my history final, i have an 86.4 and hern said he would give me the 86.5, so for sanity's sake, lets say i have an 86.5 in history for the year

i think you're right
even a 100 would only get you an 88.5 or something in that area

it could affect your GPA i guess if you got the 88 instead of the 87, but i dont know whether the transcript only says "B+" or whether it's numerical.

I'm pretty sure transcripts say B+ cuz in the agenda on the page about how they do GPA it's in terms of letter grades.

yes, what you have shown was 90% of your final grade, not ninety percent of the final exam added on to your final grade. if you got a ninety it would bring your grade up, and not down.
