When the thought of going back to work induces a panic attack so severe it can only be quelled by medication, I'd say it's time to quit that job, n'est ce pas?
If I really wanted to continue I would take a medical break...and hiatus and get serious counseling...and see how I feel after that...
BUT that being said you are doing one of the hardest most stressful jobs available and it is HARD when you are a person who actually gives a shit about people, animals and the world.
You know what is best for you though and if you know that the stress is NOT going to go away if you rest for a while yeah totally time to find less stressful work.
I still have that candle lit for you...keep us posted...
I am thinking about taking and EMT course but only for times like parties and conventions...not for everyday that would be totally too much for me..
Be proud that you managed to last this long AND go the extra mile for many people and animals...
I loved my job but the envornment started to cause both my wife and I to have panic attacks at our last jobs. It got to the point that rounding the turn to work would start making her want to throw up and cry. I stared dreading the thought of getting in my car every day.
But I loved the work we did. I ultimately quit, and so did she a few months later. Despite how we felt about the work we did (nothing nearly as intense as yours) getting that stress out of our lives improved everything for us, including our relationship.
You either need an extended leave and some counseling is suggested previously, or yes, it may be time to switch lines of work.
Or cling by your fingernails for as long as possible, alienating as many of your co-workers as you can, until they fire you?

Even with the anxiety of being unemployed and all, I feel so many, many times better not having to go back there. So yeah, if it would make you happier, get out of there.
FWIW, when my own job with the Shitty, er City, had me vomiting until my stomach was empty whereupon the dry-heaving started every time I had to go in there, I started planning an exit strategy which I will hopefully be implementing in the next few months. You're so not alone. Being any kind of first responder in this town is all-horrifying, all the time.
