Actually, that's up to interpretation. I first thought of it as Wilson asking House to kill him, but the final version reads like Wilson offed himself in front of House.
Actually, I was going for him waking up symbolically but after reading it over, dreaming works too. (Depends on if you like fluff or angst I guess)
If you don't mind me asking, what freaked you out? It wasn't actually scary, I hope xD!
