What I would do is nothing. What I think I should do is more debatable. On the one hand there's what you mentioned. On the other, unless the guy has no friends/family that he sees at all regularly, I'm sure it would've come up at some point so he's at least aware of it.
Do nothing. As you said its impossible to miss so he's obviously aware of it. If he's aware of it and doing nothing then mentioning it isn't going to change his course of action. Additionally drawing attention to it might be embarrassing for him.
Agreed with other posters. But, I do not really agree that common sense tells people that. He may have no idea what it is, and we don't really either. I'd only say something if I were a dermatologist or oncologist and it was my area of expertise and I knew for sure the thing was cancer, and even then I wouldn't do it in public. er...at least not on the train.
