Dean has always been in love with Sam and Sam's pregnant body always drove him crazy in a good way, especially how his chest would always fill out with milk and seem to expand with each subsequent pregnancy. Sam is constantly pregnant but doesn't mind much because he loves their kids and Dean and also how Dean keeps packing on sympathy pounds which he loses less and less of with each pregnancy. They have a massive family... eight or nine kids... and Sam has been left with massive breasts as a result

Sam/Dean AU double mpreg weight gain multiples

Non-hunter AU (Sandoververse or any other AU). After lots of trying with no success Sam and Dean go to a doctor for help conceiving where they're told that Dean is simply too fat to conceive. He tries to diet but after three months of trying he weighs-in to discover that he has actually gained weight. Dean gives up and they decide that Sam should be the one to carry and soon Sam is pregnant. A few months into Sam's pregnancy and Dean switches from sympathy weight gain to ballooning and Dean discovers he's actually pregnant as well... with triplets. He continues to balloon throughout the pregnancy unable to help himself and not willing to trust the doctors anymore and an already fat Dean ends up getting massive. Bonus points if his eating habits rub off on Sam.

Omega pregnancies are different from female pregnancies because Omega pregnancies are very rarely single birth or even twins and are typically full litters ranging anywhere from triplets to double digits. Sam and Dean are happily together when the Omega (author's choice) becomes pregnant. They opt out of finding out exactly how many babies they're expecting (hoping it to be a pleasant surprise) but figure it's a proper litter when the pregnant partner begins showing almost immediately. People in the office have pools about how many babies and just how big they're going to be by the end since they begin to balloon quickly. Bonus points if the Alpha packs on a sizeable sympathy gut as well before it's time for the massively pregnant Omega to pop.

Sam happily pulled up pictures on his cell that he’d taken that morning: Dean’s exhausted but happy face, a shot of the belly from as far back as he could get for full effect, and a shot of him both kissing Dean’s protruding belly button and getting kicked in the chin by one of the kids. He also relayed that the doctors were certain Dean had now reached a full forty weeks, possibly more, and that yes, they had taken out a wall, but that was to expand the room they were going to use as a nursery. Not enough fics where Sam and Dean are both heavy. They’ve both settled down and gotten fat with one very significantly fatter than the other. On Halloween the smaller one happily reveals that he just found out he’s pregnant.

Teenage Sam (14 - 17) has always been really fat. He and Dean have been messing around for a while now and Dean manages to get Sam pregnant. Since Sam has been progressively getting fatter for a while neither one of them realize it for a long time and Dean has maybe even been getting off to how much fatter Sam has gotten lately and how he’s starting to get a really big rack.
