Oh noes, poor Sam! Ezekiel being so gruesomely efficient, good stuff - killing rather than exorcising, like he doesn't give a damn about the possessed people... and if he's already consumed Sam, looks like he doesn't. Nice one :)

Things have been good between Sam and Dean, except for the obvious spoilery issues. They're hunting, and making a life in The Bunker, cooking and tending the library and allowing themselves to change, soften, enjoy it. They even have other people who know they're hunters and perhaps other things - Cas, Charlie, Kevin.
When out of the blue, John drops out of heaven and into their lives. With a world of darkness and grittiness, all season 1.
Dean and Sam have to deal with showing him their life, the clash between what has become their world and what used to be, and figure out how to be with John now.

Now, he knows for certain that what's in the passenger seat isn't just Sam, and Dean has no-one to blame but himself. And no-one to tell.
