I think it might be better set in Africa, where penis-stealing is a more wide-spread belief, but it sounds like a good game. If you're set on doing it in Korea, don't forget that the player should be able to sneak an electric fan into a closed room with his intended targets to kill or stun them, which makes penis theft easier.
When an inevitable crazed mob forms, is there any way to hide from them or deceive them into going after somebody else? It seems like a lynch mob of concerned men whose penises were stolen could lead to GAME OVER.
I was actually considering Sudan as a possible alternate country to set the game in, but now that you bring up fan death, Korea it is.
The crazed mob could actually be an interesting gameplay element. They are obviously a danger, but if you are crafty at hiding, you could pick off men from the crowd in hopes that some still have their junk intact, or even steer them towards your similarly-attired opponent.
