I don't know about the handprint. It's still there in Heaven and Hell. Faded and nowhere near as angry red as when we first see it, but still, it looks nasty and not like it would go away all together. Then again, HAH takes place what? two months after Lazarus Rizing? I'm no expert on burn wounds, so maybe it would stay sore and raised for some time and then fade completel?
There's also the possibility that the boys were rehymenated every time Cas brought them back to life.

So, thinking back to my own second-degree burn...which was a LONG time ago..I remember that after a month it was still there, but it was pretty gross. After two months...I'm not sure. Hmm...we need a doctor to weigh in. But seeing as how Anna could put her hand on it, I'd say that it DOES lean more towards "permanent scar".
As you say though, Castiel did heal Dean at the end of Swan Song, maybe he erased scars too? *shrug*
I never thought of that sort-of smile Castiel gave the brothers (Sam?) as indulgent and a bit satisfied but that fits. Totally loved Sam making fun of Castiel's greeting and well, I like hearing about Castiel's other form.
