I really do agree with everything you just said. I guess the cold could be cold water, or just a cold room at the nuthouse. Or the shivery cold of being in shock. I mean that kind of trauma would send you waaaay into shock.
Hee! It's a great Gene emo song. I actually love that "Same Old Scene" and "Vienna" were reused in this episode. They were both crucial in the first one, it just gave even more credence to their importance here.
Honestly, besides the fact that Keeley broke my heart in that entire section, it was all just edited with such extreme perfection, and they hit those dramatic beats SO WELL.
The editing and directing in this episode were phenomenal! I like your focus on the meat-locker foreshadowing, my take is that it's both a foreshadowing of that moment and a synaptic connection forward - though yes Alex could have been shot and could have fallen into the Thames.
I agree that the clown isn't evil. He could stand in as consciousness though I'm not sure what we have here is a Peter Pan Captain Hook/Mr Darling trope because I don't think her absent father is the latent danger/evil that under pins this.
