Crop your base to 100 x 100 pxls. Use Auto Levels and Auto Contrast.
Paste this gradient and set it to softlight.
Paste this gradient and set it to soft light
Duplicate your second gradient, and rotate 180*, so it looks like this. Paste it on, and set it to hard light
Paste this light texture and set it to hard light.
Duplicate your base and move it to the top. Erase the blue background, so it looks like this. Set it to soft light.
Duplicate your base and move it to the top. Erase the blue background, so it looks like this. Set it to soft light.
Paste this texture by inxsomnix and set it to multiply, 30% opacity
Add a text brush (this one is by terra_katta), some text (mine is Book Antiqua, size 4, with a space between each letter), and a border. And there's your icon! :D
