It's the first time Sam really understands that Dean's not invincible, and his whole world shifts.

That is just brilliant. Well, the whole thing is brilliant, but that one line...breathtaking.

This is wonderful. Pretty much perfect in all respects, what with the feeling and the slow discovery, and all these moments from the past each with something telling inside.

"It just means he loves you." "Oh." Sam shuts his eyes, suddenly feeling very hollow inside.
That sudden recognition he feels just makes this scene.

but that is the thing that knocks his breath out of him. It's the first time Sam really understands that Dean's not invincible, and his whole world shifts.
The feeling of awful stillness left behind in this section is perfect. It's unclear whose 18th birthday this is, though. It says Dean's right now. Correct?

he wants to be able to protect Dean like Dean protects him, because that's what brothers are for. Dean told him so.
Love how you work backwards through the years, and the language in this section is so right for a younger child.

"Never." Dean's eyes are closed and he shakes his head. "Always. I don't know."
So perfect it kills me.

Oh, and I don't know why I totally forgot this part the first time:

But the thing is, that wasn't it at all. What Sam really wanted was for Dean to admit that he needed Sam back. Sam wanted Dean to ask him to stay.
Dean hadn't.

That is the one thing that would explain how Sam could leave Dean and still love him-- because it's Sam who feels unneeded. And you showed that so well here.

Just lovely, all of it.
