You have to just force yourself to run. Force yourself to conquer what it is that you're afraid of. As always this advice coming from me is hypocritical and possibly ironic, but try to not think about that.
Otherwise you need to try and find a way to make it fun again. Don't think of it as training, you'll end up psyching yourself out. Run for the fun of running. Don't worry much about mileage or time, just run.
I know all about not feeling like running after work...it's a real drag to get motivated for. At the moment I wouldn't pay so much attention to how far you are running as much as just making sure that you ARE running. Try to build up from there. Don't let the nerves get to you, because you know as well as I do that as long as you want it enough, the adrenaline will be there for you on the day of the race to carry you through. But most importantly you're not a failure unless you let the failure mindset take over. Keep your mind clear, put your head down, and get running! :)
