I can handwave an awful lot of his insanity being due to the Mirakuru, really I can. But not the part where Oliver magically becomes the root of all problems. Oliver didn't kill Shado. Ivo did. Oliver making a conscious decision to kill Shado? TOTALLY worth seeking revenge over. But Island!Oliver is just not that smart, or that ruthless

It could be a case that Slade's (entirely Mirakuru-driven "reasoning") is that Oliver had Shado's affections - something Slade wanted when he was sane and then amped up to 11 under the Mirakuru - and let her die. Ivo killed her, but Oliver allowed it to happen while actively choosing to save another woman - he threw Shado - away.
So that's jealousy-mitigated-by-twisted-nobility, plus betrayal by the guy he was starting to think of as a brother (not that long after the Billy Wintergreen thing), throw in some guilt over the fact Slade did the right thing and didn't pursue Shado, when he could have because apparently Oliver loved another woman the whole time... Then add the fact Oliver can't articulate, because, Oliver and ... yeeeeah.
I can see that. I'm not saying I like it, but then I don't get the impression we're meant to. And I guess, as far as a man-pain vehicle goes, at least it's honest in the sense that Slade is totally unhinged and clearly using Shado's death to justify some horrible things that we know she would kick his
