Thank teh interwebz and their contribution in eliminating formality (or even measured informality), not that there was much in American English to begin with.
As a total random aside, I like the way the Arabic language does measured levels of formality in names.

Please take the hint. When I email you with the 'William' signature and you respond with "Hey Bill," it causes me to roll my eyes. I daresay my eyes have done more than a 360 this morning alone. We all have preferences for what we like to be called. Phil wouldn't appreciate being addressed as "P-Dawg," Michael wouldn't enjoy people calling him "Mikey," and heaven forbid you accidentally call Steven "Steve."
