Depends; cheap diamonds look pretty fake, too. There are a number of synthetic clear stones that can't be distinguished from diamonds with a casual glance, such as Diamonique and Moissanite.
Proper fakes (as opposed to pieces of glass) are as you note apparent at a glance but sophisticated fakes like cubic zirconium are quite difficult to discover. You can detect it with ultraviolet light, by scratching things that are 9 or so on the Mohs hardness scale, or with a loupe, and discovering synthetics requires very expensive tooling.
