That is illogical. No civilization can flourish without some form of support or hierarchy. If this region possesses citizens, then I presume that this is a civilization. What you are saying implies that this place has no law enforcement units.

I would not presume to be as familiar about anatomy as a doctor is required to be, but even with my limited knowledge, I believe that one can survive losing an extraneous appendage. With the proper medical supervision, of course.
