Down in the comments though, I found what are now a few of my new favorite theories about the shocking last few minutes of the ep.

Libby had known Hurley before because they were in the mental ward together.

What I don't understand is how she got from there to airline stewardess. Or was that her previous job before institutionalization and she was reinstated? Or am I misremembering that she was a stewardess.

Wasn't Cindy the stewardess? Libby said she was a psychiatrist. At least that is what I remember. I agree that Libby knew Hurley from the same mental ward. What we don't know is why she was there in the old time line.

I love theories 1 and 2.
