My husband and I had to make that decision about 1 1/2 years ago. You need to speak to a mechanic you trust and find out a) if the car is worth pouring money into.
Our mechanic, who didn't do transmissions, felt that our car was a really decent car and that it potentially was worth saving. (It didn't even have 100 miles at the time, so I'm not sure why the car even went!)
b) You need to find a transmission guy who has an excellent reputation - someone who specializes in it. You need to get a good warranty. Since we had an in with my dad's friend, we ended up getting 18 months - 6 months more than they normally give. But most places only gave 6 months on the transmission. So I'd consider a year warranty to be the minimum. Otherwise, it doesn't seem to pay for the cost.
We did this on a Nissan Altima (1997) and we paid nearly $2,000 for it, but we're still using it.

Hopefully, you can get a thorough assessment before making the next step. I know the whole "the devil you know" adage really applies when it comes to the great unknown world of cars, so may the auto gods be kind and give you a sign. Something carved in stone?
