I have a hard time with this because I know what my reaction would be, but it probably wouldn't work in your family. I would just bypass your mom after telling her and my dad that this is not a matter of fault, it has to do with health and if no one else is going to be a grown-up, then I will be. Then make the appointment for my dad myself, take him myself, pay for it myself and prepare for the consequences later. I know how complicated your family situation is so I hope it works out. But there does come the point where health comes before family politics and I hope your mom realizes that.
