How do I make a resume? What categories are there and what sort of activities are applicable for inclusion? How far back in age can/should I make reference to activities?

I'm applying for a position at a summer camp type thing with 4th and 5th grade smarty-pants kids and they want me to give them a list of activities I could do with the kids for an hour and a half every day.
