It's just when their timelines intersect. Her first time was his last and vice versa. It makes it all the heartbreaking because you know they're both running out of time and their memories will never match :(
Oh, I love the poignancy of it -- the kiss that she knows is her last because it's his first. But ... why does she know it's her last? Why shouldn't they meet, say, an hour later in her timeline (and it wouldn't be in her diary because she hasn't gotten there yet) and six months later in his timeline?
Because there are events in their timeline that are fixed points in time (tragic, ending events) so the last time will always be the last, just as the first time will always be the first. It's a lot of timey wimey stuff.
