It doesn't happen to me nowadays, but when I was a kid any costume character within twenty miles was guaranteed to pick me out of an audience. Mom confirms this. I have no idea why it was always me, but they left me with a blinding terror of being caught off-guard in front of a crowd.
I've only been to one "interactive" theater performance, and all that seemed to mean was standing for two hours and being pushed a lot. I can't say it was my favorite, but it's not anxiety inducing for me.
I'm only commenting in support, since you know I feel the same way. I hate it so much that I won't even GO to interactive theater things. And God forbid I win a raffle or something. I DO NOT WANT TO GO ON THE STAGE.
And if they say, "Can I get a volunteer," let me tell you whose hand will not be up.
I don't like it if I am surprised by it. If I know what it is going in I am fine, but getting surprised by that sort of thing annoys me. I however not dealt with it much. Partly because I am little paranoid and will not go to a social situation where I don't know exactly what it is and what the "rules" are. That is sort of a sad commentary on me but I never professed to be one of the cool kids.
It has nothing to do with the cool kids. It's disorienting for anyone to be called out in a public situation without knowing the rules. The rules let us know how to behave.

My company did a Murder Mystery Theater thing one year, and I wanted to crawl under a table for the entire performance. Hate it. haaate it.
