There's a joker who was just horribly mutilated in a freak accident who always looks like that.
Then there's the incarnation you saw in the movie. He just shows up on a pale horse out of nowhere. In this version, it's self mutilation, but he makes sick jokes telling people a different story about the scars each time.
Then there's another version where he just has a rictus grin all the time and his face more or less got stuck like that.
The joker is more of an idea than a person, the opposite side to batman's coin, which is why you can imagine a mirror sitting between them on the table in the scene with the interrogation room in the movie.
I loved the movie too...it scared the shit out of me at points, but it was such an awesome ride! So, Andy told me (during the movie, since we felt the need to chat about these things) that the story about his wife was pretty close to what happened. The joker had a wife, and she really did have a gambling problem (part of how he came up w/his moniker), anyway, she kept borrowing money to feed her habit and a loan shark cut her face like that after she couldn't pay back. She went crazy after such mutilation and either cut his face or he cut his own out of sympathy. Maybe. Sigh..what a mystery!
