I've heard of so many different results from cortisone shots - from perfect results to nothing. Why is that? Fmg got a shot in his ankle but the results only seemed to last a couple days.

It's quite variable - mostly because we can only guess if you have active inflammation in the joint, or if it's just degenerative arthritis. If it's degenerative you may get some benefit, but it won't last long. If it's actively inflammed the cortisone (If injected in the correct location) will definitely calm the inflammation down, and should hopefully last 6 to 12 months.

Some joints - particularly ankles - are difficult to inject in the correct location, so my guess is that the cortisone may not have actually got into the joint, but went into the surrounding area.

Has he had a bone scan? That will show active inflammation. The other thing to consider is an MRI, or arthroscopy.
