It's difficult to say. This may not be the earliest documentation. Worse still, there could be countless years of undocumented occurrence. It appears as though there are several hundred years worth of names and dates, however.

It would seem the Natives' knowledge is limited in the way that they have only the information passed down to them, or that which is gleaned from books to go by. A little like ourselves.

I would wager that they are, but such suspicions, whether warranted or otherwise, will not appropriate progression for the analysis of the Door and how it operates.

The documentation aside, the rest of what we have to go on is, at best, guesswork. It is believed that the Mist or the pair known as the Twins are responsible for the Door's behavior, but without evidence of such a fact, it's difficult to say for certain.
